Professional Recovery Network Evaluation General Information

After we have made an appointment for your evaluation, please follow these simple steps:

1. Let PRN know that you have set up an appointment with me. They will then forward information to me that I need prior to our meeting.

2. Fill out the pre-evaluation forms from my website. (about 10 pages)

3. Bring the completed pre-evaluation forms with you to the appointment.

After you call, I will work with you to set up an appointment as soon as possible. When you arrive, you will be taking a SALCE written evaluation. S.A.L.C.E. stands for Substance Abuse / Live Circumstance Evaluation Survey. It is a Drug and Alcohol screening test. The evaluation consists of 98 questions. The answers are written on the evaluation sheet itself. Most questions are “yes” or “no”. After you complete your answers, I put them in the computer and the computer generates a response. The response evaluates your ability to tell the truth, the level of stress you’re experiencing, and your level of use with alcohol and drugs.  In addition, there is an interview, where we review the forms you have filled out and brought with you. There may be additional drug and alcohol evaluations.

How To Schedule An Appointment for Professional Recovery Network Evaluation

All appointments for this evaluation are made via phone at 713-524-9525. If you are not able to make your appointment, please call me and give notice as soon as possible.

Instruction to take the Professional Recovery Network Evaluation:

Please be on time. This avoids extra costs, delays, and having to make additional appointments. My suggestion is that you aim to get to your appointment early. It takes approximately 25 minutes to fill out and review the SALCE, and another 30 minutes for our interview. Therefore, the entire Professional Recovery Network Evaluation takes approximately one hour. Bring the completed pre-evaluations forms with you. Payment can be with cash, money order or credit card. (no AmEx or personal check) Once completed, I will send your evaluation and formal report to PRN usually within one week.
